Foyer des Pionniers is seeking a dedicated Nurse Practitioner to join our collaborative, interdisciplinary long-term care home in Hearst.  This permanent full-time position offers the opportunity to provide comprehensive person-centered care, with a particular emphasis on supporting medically complex patients in achieving their goals.

Key Responsibilities:

• Provide the full scope of primary care (assessments, diagnosis, treatment, ongoing health management) services to residents.
• Collaborate effectively within an interdisciplinary team environment.
• Focus on managing residents with complex medical needs to help them meet personalized care objectives.

Qualifications:

• Nurse Practitioner Certificate (Extended Class)
• Current registration in good standing with the College of Nurses of Ontario
• Membership with the Nurse Practitioners’ Association of Ontario (NPAO)
• Experience in a long-term care setting is an asset
• Proficiency with Electronic Medical Record (EMR) systems
• Bilingual
